Description:
Janumet XR combines sitagliptin, a DPP-4 inhibitor, and metformin, a biguanide, to improve glycemic control in adults with type 2 diabetes mellitus. Sitagliptin enhances the body’s own ability to lower blood glucose by increasing incretin levels. Metformin reduces glucose production in the liver and improves insulin sensitivity.

Indications:

• For adults with type 2 diabetes mellitus to improve blood sugar control alongside diet and exercise.

• Suitable for monotherapy or as part of a combination therapy regimen.

Dosage and Administration:

• Typical dose: One tablet once daily with the evening meal.

• Tablets must be swallowed whole with water; do not split, crush, or chew.

• Adjust dosage based on patient’s glycemic response and renal function.

• Maximum daily dose: sitagliptin 100 mg and metformin 2000 mg.

Contraindications:

• Type 1 diabetes mellitus

• Diabetic ketoacidosis

• Moderate to severe renal impairment

• Known hypersensitivity to sitagliptin, metformin, or any component of the tablet

Warnings and Precautions:

• Monitor kidney function before and during treatment

• Risk of lactic acidosis, especially in renal dysfunction

• Use with caution in liver impairment or heavy alcohol use

• Not recommended during pregnancy or lactation

Possible Side Effects:

• Nausea and diarrhea

• Headache

• Respiratory tract infections

• Hypoglycemia (especially with sulfonylureas or insulin)
