Therapeutic Class: Potassium-Sparing Diuretic / Aldosterone Antagonist
ALDACTONE is indicated for the treatment of:
Edema associated with congestive heart failure, liver cirrhosis, or nephrotic syndrome
Hypertension (high blood pressure), alone or in combination with other antihypertensives
Primary hyperaldosteronism (diagnosis and treatment)
Hypokalemia (low potassium), especially when other diuretics cause potassium loss
Heart failure (NYHA class III-IV) to reduce morbidity and mortality
Spironolactone is an aldosterone antagonist. It acts on the distal renal tubules to inhibit sodium reabsorption and potassium excretion, resulting in a diuretic effect while conserving potassium.
Edema/Hypertension: 25–100 mg daily in single or divided doses
Heart failure: Typically 25 mg once daily (may be adjusted based on serum potassium)
Primary hyperaldosteronism: 100–400 mg/day
Dose should be individualized based on clinical condition and renal function
Monitor serum potassium and renal function regularly
Risk of hyperkalemia, especially in patients with kidney dysfunction or when combined with other potassium-sparing agents
Use cautiously in elderly or diabetic patients
Contraindicated in patients with anuria, acute renal insufficiency, or hyperkalemia
Common: Hyperkalemia, gynecomastia, breast tenderness, dizziness, gastrointestinal upset
Rare but serious: Severe hyperkalemia, renal dysfunction, allergic reactions
Store at room temperature (below 30°C) in a dry place. Protect from moisture and light. Keep out of reach of children.
