Indications:
ZOCOR is indicated for:

• Reducing elevated total cholesterol, LDL-C (low-density lipoprotein cholesterol), ApoB, and triglycerides

• Increasing HDL-C (high-density lipoprotein cholesterol)

• Treatment of primary hypercholesterolemia and mixed dyslipidemia

• Prevention of cardiovascular events such as myocardial infarction, stroke, and need for revascularization procedures in patients at high cardiovascular risk

Mechanism of Action:
Simvastatin inhibits HMG-CoA reductase, the enzyme responsible for the rate-limiting step in cholesterol biosynthesis in the liver. This leads to a reduction in cholesterol production and improved lipid profile.

Dosage and Administration:

• Recommended dose: 5–40 mg once daily in the evening

• The 40 mg dose is typically used when lower doses are inadequate to achieve lipid goals

• Should be used with a cholesterol-lowering diet

• Dose adjustments may be necessary based on lipid levels and tolerability

Warnings and Precautions:

• Contraindicated in active liver disease and during pregnancy or breastfeeding

• Use with caution in patients with a history of liver dysfunction, muscle disorders, or those taking interacting medications (e.g., certain antifungals, antibiotics)

• Monitor liver function tests and creatine kinase (CK) if muscle symptoms occur

Side Effects:
Common: headache, abdominal pain, constipation, nausea, muscle pain
Rare but serious: myopathy, rhabdomyolysis, elevated liver enzymes
