Indications:
ZOCOR is indicated for:
Reducing elevated total cholesterol, LDL-C (low-density lipoprotein cholesterol), ApoB, and triglycerides
Increasing HDL-C (high-density lipoprotein cholesterol)
Treatment of primary hypercholesterolemia and mixed dyslipidemia
Prevention of cardiovascular events such as myocardial infarction, stroke, and need for revascularization procedures in patients at high cardiovascular risk
Mechanism of Action:
Simvastatin inhibits HMG-CoA reductase, the enzyme responsible for the rate-limiting step in cholesterol biosynthesis in the liver. This leads to a reduction in cholesterol production and improved lipid profile.
Dosage and Administration:
Recommended dose: 5–40 mg once daily in the evening
The 40 mg dose is typically used when lower doses are inadequate to achieve lipid goals
Should be used with a cholesterol-lowering diet
Dose adjustments may be necessary based on lipid levels and tolerability
Warnings and Precautions:
Contraindicated in active liver disease and during pregnancy or breastfeeding
Use with caution in patients with a history of liver dysfunction, muscle disorders, or those taking interacting medications (e.g., certain antifungals, antibiotics)
Monitor liver function tests and creatine kinase (CK) if muscle symptoms occur
Side Effects:
Common: headache, abdominal pain, constipation, nausea, muscle pain
Rare but serious: myopathy, rhabdomyolysis, elevated liver enzymes
