Description:
Janumet XR combines two oral antihyperglycemic agents: sitagliptin, a DPP-4 inhibitor, and metformin, a biguanide. Sitagliptin improves glycemic control by enhancing the effects of incretin hormones, promoting insulin release, and reducing glucagon levels. Metformin works by decreasing hepatic glucose production and improving insulin sensitivity.

Indications:

• For adults with type 2 diabetes mellitus to improve blood glucose control when diet and exercise alone are inadequate.

• Suitable as monotherapy or in combination with other antidiabetic agents.

Dosage and Administration:

• Usual starting dose: one tablet once daily with the evening meal.

• Dosage should be individualized according to the patient’s clinical profile and kidney function.

• Do not crush, split, or chew the tablets.

• Maximum daily dose: sitagliptin 100 mg and metformin 2000 mg.

Contraindications:

• Type 1 diabetes mellitus

• Diabetic ketoacidosis

• Moderate to severe renal impairment

• Known hypersensitivity to sitagliptin, metformin, or tablet components

Warnings and Precautions:

• Monitor kidney function regularly

• Risk of lactic acidosis, particularly in renal impairment

• Not recommended during pregnancy or breastfeeding

• Caution with alcohol intake and liver disease

Possible Side Effects:

• Nausea

• Diarrhea

• Headache

• Hypoglycemia (especially with insulin or sulfonylureas)

• Respiratory infections
