Description:
Janumet XR is a combination of sitagliptin (a DPP-4 inhibitor) and metformin (a biguanide) designed to help manage blood glucose levels in adults with type 2 diabetes mellitus. Sitagliptin enhances incretin levels to increase insulin release and decrease glucagon secretion. Metformin lowers hepatic glucose production and improves insulin sensitivity.
Indications:
For adults with type 2 diabetes mellitus to improve glycemic control when diet and exercise are not sufficient.
Can be used as monotherapy or in combination with other antidiabetic agents.
Dosage and Administration:
Recommended dose: One tablet once daily with the evening meal.
Dosage should be individualized based on the patient’s current regimen and renal function.
Tablets must be swallowed whole; do not split, crush, or chew.
Maximum daily dose: sitagliptin 100 mg and metformin 2000 mg.
Contraindications:
Type 1 diabetes mellitus
Diabetic ketoacidosis
Moderate to severe renal impairment
Known hypersensitivity to sitagliptin, metformin, or tablet components
Warnings and Precautions:
Monitor renal function before and during treatment
Risk of lactic acidosis associated with metformin
Not recommended in pregnancy or during breastfeeding
Use with caution in hepatic impairment or alcohol abuse
Possible Side Effects:
Nausea
Diarrhea
Headache
Upper respiratory tract infections
Hypoglycemia (especially when used with insulin or sulfonylureas)
