Description:
Janumet XR is a combination of sitagliptin (a DPP-4 inhibitor) and metformin (a biguanide) designed to help manage blood glucose levels in adults with type 2 diabetes mellitus. Sitagliptin enhances incretin levels to increase insulin release and decrease glucagon secretion. Metformin lowers hepatic glucose production and improves insulin sensitivity.

Indications:

• For adults with type 2 diabetes mellitus to improve glycemic control when diet and exercise are not sufficient.

• Can be used as monotherapy or in combination with other antidiabetic agents.

Dosage and Administration:

• Recommended dose: One tablet once daily with the evening meal.

• Dosage should be individualized based on the patient’s current regimen and renal function.

• Tablets must be swallowed whole; do not split, crush, or chew.

• Maximum daily dose: sitagliptin 100 mg and metformin 2000 mg.

Contraindications:

• Type 1 diabetes mellitus

• Diabetic ketoacidosis

• Moderate to severe renal impairment

• Known hypersensitivity to sitagliptin, metformin, or tablet components

Warnings and Precautions:

• Monitor renal function before and during treatment

• Risk of lactic acidosis associated with metformin

• Not recommended in pregnancy or during breastfeeding

• Use with caution in hepatic impairment or alcohol abuse

Possible Side Effects:

• Nausea

• Diarrhea

• Headache

• Upper respiratory tract infections

• Hypoglycemia (especially when used with insulin or sulfonylureas)
